Questionnaire of the applicant

The disclosed information is subject to regulations on personal data confidentiality (Act No. 101/2000 Coll.). By sending the questionnaire via e-mail, I agree that my personal data are used for the purpose of keeping a record of job applicants and that, in case I am employed, they will become part of my personal documentation. In case I am not employed, the questionnaire will be withdrawn from the database after the period of 1 year or, if I wish it to be withdrawn earlier, I will send you a written notice.
